The midline doesn't do much - it's a basic trip computer.

The highline, on the other hand, does a lot of other things, such as setting how long the lights stay on after you open/close the locks using the remote.

You get many more options with the highline - but its stuff you set once an dthen leave after that!
The reason why I went for highline is for when I get the RNS510 at some point, so I can see directions etc on the highline computer. ;)

The highline addition is quite expensive I think - sure I read somewhere it needs a dash replacement as well! Probably not worth the bother TBH, unless you will really use it for Bluetooth or sat nav etc.

I dont think you have to hav ethe MFSW to operate the Highline - you can still use the stalk as you do on the midline. The MFSW just makes things much easier as all the buttons etc are on the wheel within easy reach of your thumb!
On the right side you have the Menu, OK, and Up/Down buttons - which are all used for the Highline.
On the left side you have Volume up/down buttons, a phone button (if your have VW Bluetooth installed), and a button which mutes the audio/radio.

I've got the Audi equivalent of highline in my DIS. And tbh, once the novelty factor wears off..... well, you kind of ignore it. I usually use mind to find out what my range is. Nothing else tbh.
It's a gadget  but certainly not a deal-clincher.
When I get my Satnav/stereo (I'm ripping the whole lot out and putting in a new system - the VW stuff is sh@t), I'll use a fookin big screen instead!! LOL!!
