If you have the later golf 90+ then you need to get pads, calipers and brake hoses. I think earlier golfs will need a hand brake cable as well, you can tell if you have the later ones if the hand brake cable goes under the caliper.
I managed to get everything from c&r enterprises. It wasn't the cheapest way of doing it but I was fed up with the girl friend saying "the hand brake was not working".

Then you simply unbolt the old calipers leaving the carrier in place as the mk4 carriers do not fit, and bolt the new ones on.

I would strongly recommend getting the tool to wind the calipers in from halfords or such like, and make sure that the new calipers are wound right in. That way they auto adjust correctly.
