Yep that is the only difference, second one is reinforced to handle more weight. Which means it rides harder than the other.
Unless you need it I'd go for the one with 91 load index - with this being a pretty low profile tyre it won't flex and flop too much in corners anyway.

What is the best tyre size to get for road performance?
I currently have on 225/45/17 and as I'm buying 4 is this the correct size to get?
