Alternator belt won't stay on

Well, I seem to be having trouble holding on to alternator belts. On my 3rd one now - garage says that the pulleys seem to be mis-aligned but I can't see it.

I notice that the adjustment bolt on the alternator isn't the ratchet type - just a normal bolt. Could this be allowing slippage?

Does it have to be the proper bolt?

No, I doubt it is the bolt causing the problem. It is quite common for those teeth to get worn and on my cab I have replaced the toothed bolt for a normal one. As long as you tension the belt ok the replacement bolt will do the same job.
